Registration for the 7 Habits for Small Business Managers in Blanding on May 16-17

STEP ONE - Determine your cost
The total cost of this program is $395 per attendee. Thanks to a training partnership agreement with Utah's Custom Fit Training program, owners and employees of for-profit businesses in Utah may attend this training program for $198 per attendee. Custom Fit Training will pay the other $197. If you are attending as the owner or employee of a for-profit business in Utah, use the button below to print, complete and sign a Custom Fit Training agreement. Follow the instructions on the form to submit the agreement.
STEP TWO - Pay the registration fee
Registration and payment in full is due by May 12. Select the appropriate invoice below and remit payment to the USU Easter Blanding Campus cashiers office. Payment may be made in person or by sending a check to USU Eastern, 576 W. 200 S., Blanding, Utah 84511. Please enclose the appropriate invoice (below) with your payment. Alternatively, you may simply call to pay by credit card: (435) 678-8100.
If you are not eligible for a Custom Fit Training partnership, use the invoice below. Examples are individuals who are not attending as an owner or employee of a for-profit business in Utah or people who are employed by a non-profit or out-of-state entity.
